For these specific match ups; here is my take:

Blue Moon
I agree with you completely about Destructive Revelry. I actually believe that some players will most likely cut some Blood Moon's post board. Blood Moon isn't amazing against us, but it isn't as bad as some may think. It cuts us off of up to 8 cards from our deck. And it also stops us from using our fetches for deck thinning and fueling Lavamancers. I do also like your plan to bring in the Searing effects. Their creatures will all die to them (TiTi won't but if it blocks it will) so they'll always be a 2 for 1.

UR Breach
Very similar but they are more likely to play morr copies of cards like Electrolyze and other burn spells. I agree with your sideboard plan for this as well for the most part. However with this deck more likely to have less Blood Moon's than Blue Moon; I would choose Helix over Skullcrack. And if you do see Madcap (some may main/side this) I would bring in the Path's if you see an Emperion.

Martyr Proc
I agree with everything here that you said as well, but I personally would bring in a copy of Path over Searing Blaze. Path is, like you said, just OK. But having that single copy at least gives you a way to deal with a big Serra Ascendent without having to 2 for 1 yourself. Searing Blaze is very disappointing without Landfall.

With your current sideboard, I agree with pretty much everything you said.
The only suggestion I would pop in is that you add some kind of graveyard hate. I personally play 2 [[Grafdigger's Cage]] and a single [[Relic of Progenitus]] (swap the numbers depending on if I'm expecting more/less stuff like Collected Company). The weakest card in your sideboard looks like Kor Firewalker as it's only good in the mirror. I'very played it for the mirror a lot and have found it to actually be really bad. It only works against one deck and usually costs you either 3-6 life just to cast. I highly recommend adding some grave hate to your sideboard.
