ITS A TRAP!!! (application questions)

Now that I have your attention

Hi everyone! First i want to mention that if any of you are on admissions board for phd schools and dont require official scores till acceptance that i love you because the costs for grad school applications are awful.

Next I had a question I wanted to examine from the perspective of academics. On many graduate applications, a common theme is to discuss academic history. I was wondering if this can be a trap question or red flag area like career goals. I know many people dont want to take on students that say they want to treat depressed individuals since they have lived the unique experience and can provide a valuable insight to experince and improve treatment outcomes for others.

For example
My associates i made a 3.52 BS a 3.62 and my grad program a 4.0.

During 2018 I had been diagnosed with carpal tunnel in both hands which had previously affected my perfomance in my undergraduate

2019 i recieved a late diagnosis for ADHD. Would it be beneficial to mention that despite these setbacks i maintained good academic standing and graduated with honors in my undergraduate course work, and subsequently improved my performance being treated for my physical and cognitive setbacks? I was originally worried about my GRE scrores but since im basically 85% in every category but math im not sure if its too big an issue since i havent had a math class other than statistics in 6 years.
