All of the villains in the Star Wars Prequel trilogy perfectly foreshadow the rise of Darth Vader. Maul was manipulated from birth by Palpatine, Dooku was a fallen jedi, and Grievous was a cyborg. Vader is all 3 of those things

In this case, balance doesn't mean equal numbers. Listen to Obi-wan in his fight with Anakin: "You were supposed to bring balance to the Force, not leave it in darkness!" At the end of Episode 3, there is no balance, only darkness with the faintest glimmer of hope in Luke. Jedi exist harmoniously within the Force, Sith are aberrations that cause imbalance. Windu talks with Yoda about the Jedi's abilities in the Force being diminished, the very fact that the Sith exist ruin the natural balance or harmony in the Force.

The prophecy really wasn't ever about balancing sides, nor was that meant as some twist that the prophecy was misinterpreted when it turned out that Anakin contributed to two Jedi and two Sith being left (which doesn't hold up, Luke and Leia were alive even if they were untrained, and the Rebels tv show has two more, plus several dark side users.)

The prophecy was correct, Anakin brought balance to the Force when he destroyed Palpatine and died doing so, eliminating the Sith and returning balance to the Force.
