How do I report a Dive shop for unsafe practices?

Assuming you’re not a troll, you should be infinitely more worried about your unsafe diving practices.

You let yourself get rushed off the boat. You chose to dive without a snorkel. You chose to dive with gear that you considered to be expired or improperly serviced or whatever.

File a report or not, but YOU are always responsible for your own safety. You should check your gear ahead of the dive - if there’s a problem, do not dive, no matter who tells you it is okay. You should have done your buddy checks - if you felt rushed, it’s your job to tell them to stop rushing you; if they insist, then don’t dive. If you feel that diving without a snorkel is unsafe, then don’t do it - no matter who tells you it is okay (it’s okay, snorkels are the fucking worst but if you need to use one, then it is your job to make sure you have one and to not get in the water without one).

You can file all the complaints you want, but you should rethink your attitude. You’ve listed ways in which you think they were unsafe, but in reality you’ve listed ways in which you - according to your own standards and judgment - repeatedly and voluntarily made bad, unsafe decisions. You are unsafe as a diver at present. If you are unwilling to assume responsibility for your own safety, no matter what, then you have no business diving unless you are under the supervision of a dive master or instructor.
