The camera angle that actually shows the start of the fight looks like Polak engages and gets rocked early at which point Nurse took it too far. Maybe he was trying to draw a penalty, maybe not but he definitely had his hands up as he deviates towards Nurse. Maybe that's why the refs didn't call the instigator because they saw the same thing in Polaks actions
