Do PVCs Increase Over Time?

The answer is 'it depends'.

I Know that isn't the most helpful thing I can say. But it's true. Some people get a baseline level of ectopics and they remain fairly stable for life. Other people feel them during a particularly stressful or challenging time of their life and find they disappear again shortly afterwards. And some people do unfortunately deteriorate a little over time too.

When you say you estimate a few thousand PVCs each day, what cardiac investigations have been done? I would hope they have given him a holter monitor to measure exactly what his burden is.

Another investigation that should hopefully be done is a blood test. There's quite a few potential mineral and electrolyte deficiencies like magnesium and potassium that can cause PVCs and be very quickly and easily resolved for example.
