Aetna will no longer be in contract with my hospital.

You would need to file a continuity of care but I think the provider has to fill it out too or if they are the ones who fills it out. I would call the insurance company and ask for supervisor and ask them about the continuity of care process.

While continuity of care means that the insurance company will process the claims as in network but the provider can balance bill you. So I would start calling the providers and ask for the office managers and see what they about that part.

The reason I am advising you to talk to any one who is a supervisor or manager as you have a very complex chronic illness and you cant have delays from people who have no clue on what you are doing.

Maybe also ask for a case manager at the insurance company sometimes they point you in the right direction and help to work with your providers.
