My apoc difficulty bleed build. I use provisioner ring instead of bloodletters when clearing dungeons.
I think it's better to just focus on either bleed or nightmare spiral. I think it is unnecessary to have both, especially with the pistol too. Devastator does best bleed damage, this can be buffed further with razorwire necklace and applying corrosive status effect (hive cannon, swarm, corrosive aura). With the relevant traits this should be enough to regenerate your health in a few seconds. You can also use dragonhearts, if necessary.
The bleed effect lasts 20 seconds, so i don't think you need any labyrinth armour or a 2nd source of inflicting bleed (melee), you have more than enough time. Lots of armour choices work here. I use leto for extra tankiness, slayer would work well with devastator. Void can also be even more tanky than Leto's in apoc. I then use Bright Steel Ring to counter the heaviness of the armour. In most cases i would commit to one armour set though. Full Oss' would work with hunting rifle better than your current mix.
Think that's everything. Nightmare spiral you would want high ROF gun, maybe oss' armour set and any trinkets which increase weapon damage or healing effectiveness (bloodfont is another which is good for both builds if you want extra healing). Mods maybe song of swords and swarm, but i have never tried that build.
The other health steal build i use is the summoner one. Soul Link + Labrinth + Burden of Follower, etc.
