Yes, as another comment points out, hindsight is 20/20, and it's awfully easy to armchair-quarterback these things...

When the officer asks the woman if there are guns inside and she answered in the affirmative... seems like at that point, it would have made good sense to back off and call in S.W.A.T.

At the very least, ballistic shields should be available to any officers executing warrants. They make pretty small, lightweight but effective shields these days. If I were in that line of work, I'd sure want one of those in my vehicle all the time.
