What is the most NSFW thing you've seen at your job?

I saw a guy get his head caved in after a series of stupid decisions on a forklift:

  1. His load was too tall.
  2. His forks were too high even before load height.
  3. His load wasn't secured.
  4. He was going too fast (in reverse) for his load.
  5. He was in the wrong lane.
  6. He stopped too quickly when another forklift (that was driving correctly) came around a corner.
  7. He jumped out of the cab as his load fell instead of staying inside it.

He was moving a load of 16 cases, each weighing around 200 lbs. These cases are only supposed to be 8 to a pallet, with pallets absolutely not double-stacked, but to save some time, he decided to pull the cases off one pallet, place them on the second without properly securing them, then dropped the whole thing on the empty pallet to restack when he got them to the destination.

When he jumped on the breaks, the top 8 cases, being unsecured, skidded back towards the cab and bounced off the framing (aka, the metal bars that exist to protect against incidents exactly like this). He'd have been fine (fired from the job, but fine as far as health goes) if he'd have stayed in the cab, but he jumped out when he saw the boxes starting to go, and one of them bounced off the frame and smashed into his head.

He survived, but had a crushed skull and severe brain damage. If any one single thing in that above list had been omitted, he would have been fine (in terms of his own personal safety). The most uncertain point in that statement is dependent on how he'd react to a forklift coming around the corner if he had been in the correct lane, but at least he wouldn't have been on a collision course and forced to hammer the breaks.

I can honestly say that if I never see the human brain in person again, I'll be quite happy about it.
