Anal rhagads & sphincter spasm [25/f/171cm/55kg/caucasian] [+ photo]

My diet is mostly plant based, I eat a lot of different vegetables, beans, mushrooms, nuts and seeds, soy products. Basically lots of fibre. Wheat products will constipate and bloat me if I consume too much. I drink between 2 and 4 liters of water and/or tea a day. Breakfast could be a serving of fruits with a little bit of granola, nuts, soy yoghurt or protein shake - lunch could be a big bowl of mixed vegetables - dinner could be a salad and fruit, or fruit with a couple of granola or protein bars. I try to have multiple meals and snacks a day but sometimes it ends up being 2-3 meals as my stomach is used to get a lot at once (I am working on it). Well, my body will clench automatically, that's why I was diagnosed with a sphincter spasm initially. I still clench from pain now but I try to relax as much as I can - But I can't control everything and my rear seems to have its own will in these regards. Allegedly the spasm isn't really there anymore (that's what the last Dr said), however my anal tears are still really bad. I personally assume the main problem is the delicate skin which will tear up every time I use the bathroom and spasms are results of the pain (I am aware that spasms can block the healing though)... Is this enough information or is there anything else I should describe? Thank you!
