Do I disclose bipolar diagnosis—that I was unaware of—three weeks before bar results released?

There's no litmus test for bipolar disorder. I know two people who spent significant amounts of time trying to find the right meds for their bipolar disorder, only to be diagnosed (in one case years later) with schizophrenia and borderline disorder respectively.

I am not a doctor, but having heard how their doctors (both the original doctors and the subsequent doctors) diagnosed it, I am pretty surprised that your doctor felt she had a firm diagnosis after a single meeting. It might be worth considering whether she misunderstood when reviewing her notes from that one meeting.

Before you go reporting something to the BLE, it would almost certainly be worth talking to your doctor about whether it's likely you have bipolar disorder, given that you have not been taking any medication for it for 3.5 years, apparently without depressive or manic episodes. If you still don't feel convinced after the conversation, there's nothing wrong with going to see a different doctor, especially a psychiatrist if your "doctor" above is not a psychiatrist.
