Older brother being a douche.

If someone calls an ambulance for someone, then that person can refuse both treatment and transport.(or either). So say you get shot. You can refuse all treatment. Or we can bandage you on scene. Or we can bandage you and transport you.

If you don't want to be bandaged on scene, we know you're probably going to bleed out and lose consciousness. So we'll stay on scene and once you lose consciousness we'll start working on you. Even if 30 seconds ago you said you didn't want to. We can also do that if someone is intoxicated or on drugs. It's kind of like our ace card. It's the last thing we can do to make sure you get proper care. Do we bill you? Yes. Does it suck that we bill you? Yes. I want it to change, then people won't refuse treatment they need.

What is something I would say you shouldn't accept ambulance transport for but maybe treatment? If you break a bone, like you're skating and fall and break your arm. Have someone drive you to urgent care. It's cheaper.
