What the hell, let's go in for the exhaustive list.

First and most importantly, from age 4 or 5 I remember masturbating while thinking of having my appendix removed. This eventually expanded to a broader fetish (in the strictest sense of the word, I have to think about it if I want to orgasm) for any sort of medical procedure, preferably stomach/torso focused and preferably done on a fat or chubby patient.

Typical medical fetish porn doesn't do it for me; thin patients or lots of focus on genitals or breasts is an instant turn-off. I prefer to imagine myself as the patient, but it's almost as good to be the doctor/nurse. • Vore, to an extent. I used to have this fantasy about being force-fed someone who'd been shrunken to pill-size, then being shrunk myself and fed to someone else. Of course this ultimately required extensive surgery to repair.
• Incest, mostly between siblings. Bonus points for conjoined twins. I once wrote a story about twin sisters who were born conjoined and cut apart -- one sister wanted to move on and live a normal life, but the other ended up trying to sew them back together.
• Age gaps or other power imbalances, like doctor/patient or student/teacher. I don't like this between straight couples, though.
• I don't exactly have rape fantasies, but rather rape recovery fantasies, where someone who has been raped is cared for afterward, or reenacts the rape with a loving partner, or learns to love consensual pain during sex. I like to think about the rape part too, but mostly it isn't arousing.
