Not to be controversial but this is partly why in many localities you are supposed to / lawfully required to dismount and walk your bike across the crossing.

If the cyclist was walking and paying attention (as you should do) and looking at the incoming traffic direction (as you should do) then he may have seen the car coming and could have missed it.

People (the car driver) are always going to do dumb shit. They could be high on drugs. They could have just had 8 standard drinks of alcohol in an hour and well over the limit. They could have a mental disorder or be otherwise out of this world. They could be texting on the cell phone and not even looking at the road or the crossing. You cant assume you are safe when you are on the roads.
