[Serious] Mental health professional of Reddit, have you ever mishandled a patient treatment? What was the outcome of your screw up?

Sorry to hear that. A couple of years ago I had a similar situation that really screwed up my life to the point I have yet to fully 'fix' it. So...I had this huge depressive crisis months after my divorce - slept and cried all day, couldn't eat, couldn't even work. I was already taking Lexapro before and due to the worsening of my condition, my doctor tripled the dosage. Eventually, I felt better and better, then too much better then overly better. Became self centered, egoistical, narcissistic and finally, agressive. My therapist told me to ask my doctor to change my medication because she thought I was "bipolar or something". But it was too late. On a given night, I had a psychotic outbreak (that's how they called it) and after a stupid argument I physically abused my ex-wife (we were trying to get back together). I don't remember much from it, just some small moments and, believe me, I am not proud at all of it. The day after, I crashed. Tried to kill myself and another doc prescribed me another bunch of mood controlling pills (that to this day, even after stopping taking it, disrupted my whole sleep process). Eventually,the first doctor prescribed me less Lexapro, I felt better and it turns out one of the causes of my depression was low testosterone, which I discovered by myself going to another doctor due to some other issues. I really don't know if this was a screw up of his part or Murphy's law taking action, but I really do think he could've handled this better.
