An extra row of teeth

All jokes aside, it's just a lack of dental care you're seeing.

Appears like a couple supernumary (extra) teeth. Pretty common in that region of the mouth. He's honestly lucky his other adult teeth appear for the most part to have erupted, albeit quite crowded, I can't really say by the picture. These often need extracted at childhood to allow the normal complement of teeth to grow into place. Should have been noticed and addressed as a child let alone now as these are nothing more than traps for food/plaque and will abrade his tongue. If he still has these at this age he either is a weirdo who likes to show off his palate fangs or just doesn't see a dentist. I'm gonna guess both the former and latter.
