Okay, this is a difficult situation to dissect. I'm not going to pretend to be a doctor and discuss how your respective disorders might play into the relationship dynami. All I can say, maybe ask, is how do you feel about him finding you unattractive? How do you feel about changing your appearance to fit his desirable version of you? Because it sounds to me that you're the one making the effort to have intimacy. The thing about relationships is both people have to give and take. If he's not doing his part and/or you don't see him doing any more in the future, then I think you owe yourself to find someone who does find you attractive and puts in effort into the relationship.
