If this person is genuinely a bad actor insofar as they are willing to utilize nursing orders for the patient’s safety as a pawn in a chess game to retaliate against nursing staff, then adding documentation to a pile that will ultimately be used to hold them accountable is a reasonable option. Your status as a neutral third party traveler will also likely lend credibility to your claims.
