God Mode...who needs a mod? RIDICULOUS Melee/Alchemy build, check it out! (Death March gameplay)

Obviously you are utterly pwning everything so GG, but..

Why Quen intensity in armor? Seems completely useless since Quen gets popped by 1 attack anyway. You could slot greater Mendings for a total of +18/s]] passive vitality regeneration. I'll grant you that isn't amazing either, or +18% elemental damage, also not amazing, but really none of the glyphs are, but Quen at least seems like a 100% waste.

Also Free Quen at combat start - seems kind of useless IMO? Stamina Regen is 100/s outside of combat anyway and it is pretty easy to cast it before initiating. I'll again grant you that none of the other armor runes are anything amazing though. Retribution seems more useful though as it least it will provide value in-combat, and punish hits on you.

Tissue Transmutation - Seems like you may as well go for extra DPS instead. You showcased the "toughest spot" you could probably be in in this game - a Hanes camp in B&W and you utterly pwnt it, so... I see why you like the safety net though. Not much else to slot there that is worth it.

How does the invigorate work? The way it was worded made me think that the health/second buffs would be transferred into +Dmg, not life leech attacks from Ekki. Is 1/s vitality = 1% damage? (capped at +50% I see) If that is the case then Greater Mendings as said above would put you at a baseline of +18% damage before your life steal kicks in. (or +27% if you ditch the armor rune for +9 more). Would at least provide more value than Quen intensity.

Euphoria at first seemed underwhleming to me, but if it calculates the +%damage after other buffs it seems like it could indeed be huge, will have to try it out. I really wasn't nuts about any of the other mutations so the straight-forwardness of Euphoria might be the ticket.
