Le Fort 1 Osteotomy Advice Needed

A LF1 is recommended when you have a recessed maxilla and is done in nearly all cases - overbite, underbite, etc.

Note - in no way shape or form and I trying to point out flaws or anything, I'm simply giving reason why I think the LF1 would be needed/beneficial.

You didn't post a ceph/x-ray so we can't see your occlusion. But, you have a long philtrum, flat lip, downward growth, and from just eye balling, it appears when drawing the line from the nasion down, not a lot passes that threshold. All these things would be corrected with a LF1, and by doing double jaw surgery with a rotation, you'd also give your surgeon ability to maximize the mandible's movement as opposed to simply relying on further chin lengthening. Also, your upper lip will lengthen as you age, so I'd caution against a goal of no gum show when smiling.

I think you'd have great aesthetic benefit from double jaw surgery. I think it'd balance your face, give projection of your lower third, and fill out the midface nicely in addition to your bite/smile fixes. I personally wish I had your preop profile going into things.

Ultimately take my comment with a grain of salt though. X-rays and the omfs will be the decisive factors.
