I don't imagine many people have this experience so I'll chime in. I had a problem with phimosis I think it's spelled. Couldn't pull back the skin all the way to see the dickhead inside. In fact for many years I wasn't aware that penises were supposed to do that, and then it all clicked that was the reason mine looked different from what I saw on the internet. I was maybe 14 the first time I saw my penis without the foreskin. I'd had girlfriends and everything before then, but no one mentioned that it was different. So the doctor first suggested a week long trial of stretching it out many times a day by pulling it back for a few minutes each day, but it was quite painful. One day I tried to leave it all day pulled back during school, but I checked halfway through the day and it was swollen huge and red. The next step was circumcision. I got an appointment and went in with both my parents. This was a big deal. My parents had just finished settling their divorce and things were still very much heated between them. They refused to talk to each other and probably would have killed each other if left alone. Anyway it came time for me to go in and the doctor asks if I'd like to have my parents in the room. I knew having them both there would be uncomfortable, so I chose one. My Mom. Yes it seems like an odd choice, but my reasoning was my mom was a medic and fascinated with surgeries. Also I gave my dad pity as I didn't think a man would want to see another man's genitals get all sliced up. Another advantage was just before they started she suggested they give me some injection that would ease my anxiety. Oh yeah forgot that you are awake during this operation. The stuff they shot me up with must have been really good. I remember everything, but the only sensation I felt was the first numbing needle. From that point on I was more concerned with the lights in the ceiling and telling mom "Hey mom look at my arm. It's just so HEAVY! I can barely hold it." Then letting my arm drop and hit me in the face. I remember asking a couple times if I could look down because I also wanted to see how gruesome it was, but everyone suggested I just stay laid down. Afterwards they wrap me parts all up in gauze and suggest I wear tightie-whities for a while with the bandages. It was pretty tender for a while and all around the outside had these black biodegrading stitches. Looked like a barbed wire tattoo but for my schlong. At the time I loved picking scabs and stuff so a few of them got pulled out. (Sorry for every man that had to read that). I now have sort of a franken-dick, but all the ladies have told me they like the extra texture. As far as answering your question, it's not much different from before. I never really had a cleanliness problem, but it was reassuring that I was more like the other American boys and like the dudes on internet videos. I'd say the only thing I miss about it was it was infinitely easier to jerk it beforehand. I've always been a dry-guy and hj's hurt more now when the girl pulls too hard.
