This is pretty tame compared to the other stories here, but years ago during a summer in college I worked at an amusement park and one of the rides I operated was the log ride that makes a big splash at the end. Since this park had a water park area, people would always ask whether they could just wear their bathing suits on the ride (since they would get wet anyway), but park rules required the same clothing as on other "dry" rides, with one exception: girls could wear their bikini tops.

One day, these two high school girls and what I'm guessing were their much younger brothers get on the log boat and one of the girls asks if she can take her shirt off. I tell her that's fine as long as she has a bikini top underneath. She then starts to take off both her shirt and her bikini top, accidentally exposing her boobs for a couple seconds to everyone in the log boat dispatch area. I probably wouldn't have seen this if it weren't for the staff worker manning the ride exit who said "woah heh heh heh" as soon as he saw what happened, causing me to look up from the ride controls to catch the wardrobe malfunction. This also made the poor girl realize what she had done, and she immediately collapsed into a human ball to cover herself and put her bikini top and shirt back on.

I felt bad for the girl, as she was clearly embarrassed, but I also felt bad for the boy (her little brother?) who was sitting next to her during the malfunction. He went from telling his sister(?) to hurry up to silently staring straight ahead with a stunned look on his face. After I dispatched their log boat, I heard the girl ask the two boys whether they saw anything, and the kid with the stunned expression said "I saw... a little" in a slightly traumatized tone. Poor kid.
