Physicians of Reddit: What's the worst injury you've seen at a routine check-up?

Back in 1999, we had an older black gentleman come in to our clinic. His wife had to basically drag him there and she told us that he'd been complaining of pain in his lower abdomen for the past two days. He corrected her and said it had only been one day, but she insisted that it had been two. So, I got him into an examination room and we typically begin by taking their vitals, so I fastened the blood pressure cuff around his arm. He was sitting with his legs crossed (which can cause the reading to come out artificially high), so I said "Sir, can you please uncross your legs for me," but he acted like he hadn't heard me. So I said "Sir, I need you to uncross your legs," and he looked at me and he had this look in his eyes and he leaned forward and in a clear, calm voice said "I need about tree fitty." Well, it was about that time that I noticed that gentleman was about eight stories tall and was a crustacean from the Protozoic era. The Loch Ness Monster. I said "Dammit, monster! Get out of my clinic! I ain't giving you no tree-fitty!"
