Fleas: Under the terms of this agreement, Client understands that all pets are required to be upto- date on vaccines, flea, tick and heartwarm preventative. If in the event the Client’s pet brings fleas into Service Providers home, a $300 flea fee will be executed and the Client privilege to boarding and daycare services will be revoked. In addition, the Service Provider is not responsible for any flea, tick, or heartwarm issues found as a result of the boarding or daycare stay.
Liability:
Service Provider shall not be responsible to Client for any injuries as a result of Service Provider's provision of services under this Agreement. Service Provider waives any right to seek reimbursement for damages to furniture and further related household items that may become damaged as a result of the Client’s Companion Animal. Further, Service Provider should not be held responsible for any injuries or illnesses as a result of this service. By signing this agreement, the Client understands a risk of injuries that could include (but not limited to) dog bites, scratches, bite marks, etc) or illnesses that could include (but not limited to) kennel cough, diarrhea, vomit and stress colitis. Any and all veterinary treatment is the full responsibility of the Client, although Service Provider may offer compensation in good faith. This indemnification includes all employees or staff of both Parties, if applicable.
Payments:
Client agrees to submit payment online to Service Provider prior to drop off for services. Boarding hours are for 24-hour periods only. If Client pick up and drop off times equal longer than 24 hours, a daycare fee will also be charged to the Client. Client understands pick up and drop off times are between the operating hours of 7am and 9pm. If Client is late for pick up, an extra day of boarding will be charged, and the Client agrees to pickup the following day. In addition, early drop offs will work similarly in that Client’s must drop off the night before and pay for an extra night of boarding. If for any reason payment is not delivered to Service Provider prior to drop off time, Client agrees to a 20% late fee charge. Cancellation policy: Client MUST inform Service Provider of a cancellation or modification to a current stay within 72 hours. Failure to do so will result in a 50% or $20 cancellation fee, whichever is highest.
Aggression: Under the event that Client’s Pet is found to be aggressive towards other dog’s/humans, Client agrees to arrange alternative care for their Pet immediately. If Client cannot be reached, Service Provider will drop off at the closest boarding facility and notify Client. Failure of the Client to respond to an emergency will result in relocation through Animal Control.

A.) I have provided YOUR NAME HERE with a full and accurate depiction of my pet’s temperament as I know it. I agree that all information provided above is accurate to the best of my knowledge. I agree to inform SITTER of any changes in the above information, including but not limited to altercation or bite incidents involving people, dogs or other animals. B.) I give permission for SITTER staff to temperament test my dog for social activities. I understand that it is at the sole discretion of SITTER as to whether my dog will be permitted to participate in social activities. I understand that initial passing of the temperament evaluation does not guarantee continued participation in social activities and that SITTER may change my dog’s activity status at any time and for any reason. I understand that certain behaviors will disqualify my dog from receiving care at SITTER outside of social activities. C.) I understand that there is inherent risk in participation in social activities, including but not limited to: fights, bites, contagious disease, parasites and emotional distress. I understand that while SITTER takes precautions to minimize these risks, they cannot eliminate them. I give permission for my dog to participate in social activities at SITTER with the knowledge that these risks exist. I hereby hold SITTER and it’s agents harmless in the event of any assumed risk during social activities.
