When a character dies, what level do you make the replacement character?

You contradicted yourself here.

Are referring to the "decent players" comment? If so that's not a contradiction. I never said players didn't have a part in the game. Terrible players are a whole other discussion. But even the best player isn't going to have fun if the world is utterly different than what he enjoys, is boring, and run by an unimaginative rules lawyer DM who never bends.

Crafting a world and providing fun are completely different tasks. The stigma that the fun falls on the DM's shoulders is so very wrong and any DM who says that, should reevaluate their methods.

Not completely different tasks at all. The world's composition and makeup (dungeon design, NPCs, etc) are a huge part of whether or not a game is fun. The DM is the one in charge. If the players don't seem invested, it's on his/her shoulders to diagnose why and take action. Sometimes players and a given DM just aren't compatible and should look for other groups, which is fine. But a good DM doesn't stubbornly steam ahead with "his style" if the players hate it. If changing things up makes the game not fun for the DM then by all means he should relinquish his DMing duties in that group and/or just find a new group to DM. Fun cuts both ways. Everyone should be enjoying the game by no means is anyone suggesting the DM be miserable for the sake of the players. This also assumes the players themselves aren't just bad. That's a whole different scenario where the DM should just find a new group.

However, you also state DM's who only care about rules are inherently bad DM's, I hate to tell you but that is the DM's job

No. It's not the DM's job to "only care about the rules". The DM often will make decisions, tweaks, and on the fly adjustments for the good of the game. If you think a DM is just there as a strict referee and nothing else then there's the problem.

Saying that bad DM's are individuals who only care about rules is a silly and weak argument.

I never said that. I said a DM who only cares about the rules is a bad DM. DMs can be bad for a variety of other reasons too. Also calling something silly and weak isn't an argument.

but a DM with a well crafted world is doing his job. If its not the world a player wants to play in then maybe they should not have bought into the game, or maybe the DM should have asked the questions to what the players want before hand. Either way, it doesnt mean he is a bad DM. He did his part, the rest is on the players to get invested and create their own fun.

There you go. DM should have asked beforehand. Hence doing his job making sure people will have fun. A "well crafted world" is subjective. It's only as good as the group's level of enjoyment. If the players still hate the game, he needs to figure something else out. If he hates the players then find new ones.
