When did your "Something is very wrong here" feeling turned out to be true?

Reminds me of one night at work.. Work for a package delivery company. We have 'load planners' who take all the cans (about the size of a VW beetle) of packages in them and enter them into a computer to make sure a plane is balanced.

Well one night we get ready to unload a plane after it lands and the pilots come out furious if not worse. Just as we are about to unload all management and mechanics are frantically telling us to not do anything. Come to realize somehow the load planner at the previous airport didn't take in account for fuel used during flight. It took off balanced but landed with what I was told in a very dangerous area of imbalance where the plane could've tail tipped any moment let alone the pilots landing with a back heavy plane. I saw the front landing gear and never seen so much chrome metal from the shaft exposed. Took 20 mins of people making sure if anything happened in their department that they weren't to blame till they had a fuel truck come and fill the plane up to balance it out again. Scary stuff.
