Find doctors

In looking at your post history, you have some really severe impairments. You live in assisted living and require a wheelchair. Your nursing home likely has some sort of doctor or physician who can diagnose your issues. If you've had ER visits recently, those diagnoses were likely given to you from an MD. Your nursing home probably also has some records, since you've been living there for a long time. The disability analyst would request these records. You should be able to ask someone at your facility what kind of records they have. Mental health diagnoses do need to be from a PhD or PsyD, so those are harder. You would likely need to see a neuropsychologist to get memory or IQ testing, unless you had an IEP in school and have those records. But your functioning appears severely impaired, so that should work strongly in your favor. If you absolutely don't have the diagnoses, DDS will set up an exam for you, but you will have to get there, which may be a lot for you physically. Best of luck.
