So how is it really like being an black African transgender woman in rural Germany ?

I mean, I din’t know what you red, but Germany seems to be a very tolerant place for black transgender women. Much more so than then the USA, with a long history of abuse of even murder which actually is sometimes big in the news (when media decides to notice it). So I think it’s as good as it gets, if you want to study in Europe or anywhere in the West. Not perfect, for sure, but generally should be OK. I understand that you are afraid, but I really don’t know what place to suggest that would be better. Maybe some Dutch or UK universities, but not small town ones, maybe some well known and expensive US ones. But as we are still not there (as the whole world) when it comes to both race and gender acceptance despite it being 2019, I think Germany is really as good as it gets. (Am not German and nit advertising it)
