I had to be prescribed narcotic pain meds when I nearly died of an internal bleeding episode from a ruptured ovarian cyst because NSAIDs would've exacerbated the internal hemorrhaging and no amount of acetominophen would've put a dent in the type of excruciating pain I was in, this type of pain was comparable to any kind of internal organ rupturing, like a gall bladder or appendix, with the added danger of severe internal hemorrhaging. The pain was so intense I nearly passed out whenever I attempted to shift positions, I could not sleep or speak, my abdomen was distended 3x it's size and my abdominal cavity was filled with 4+ pints of blood. It was becoming difficult to breathe, according to the ultrasound technician the blood in my abdominal cavity was putting pressure on my lungs.

Anyway, I couldn't take blood thinners because they would've exacerbated the internal hemorrhaging. The ER doctors even told me I could die from shock when I arrived, because I had waited nearly 10 hours hoping for the pain to subside on its own before I was brought into the ER. I had no idea I was bleeding internally until I was given a CT scan and an ultrasound. Anyway, I was given IV dilaudid that day by the ER doctor and prescribed narcotic painkillers because it was the only option for me to be able to sleep, lie down comfortably or even move without passing out from the blinding pain, and I was taken off them slowly with no issues. This happened when I was an otherwise healthy, extremely fit 24-year-old, so there are definitely cases when narcotics need to be prescribed to people who aren't terminally ill.
