I think smoking and other unhealthy habits are personal choices, and while I don't necessarily fully agree with the argument that it is primarily a 'systemic' problem, I can very easily understand that point of view. However, the fact that smoking, poor eating habits, recreational/non-recreational drugs, etc. are hard to quit doesn't really change the point I was hopefully illustrating.

I don't wish anyone who is covid vaccine hesitant any harm, there is a lot of weirdness around every aspect of Covid as it has been presented to us and every day we learn a little more. It's just a very strange position to hold so much distain for those people and argue that they should pay their own medical bills in a country where our healthcare system takes care of everyone and does not discriminate, including people with preventable illness caused by poor personal choices.
