Total Cholesterol (mg/dL) | 538 | 396 | Under 220 | Varies |
HDL-C (mg/dL) | 95.6 | 45 | Over 40 | Over 70 |

So we've seen something I would expect on keto initially:
HDL-C can drop initially and then pick back up. This isn't uncommon in the labs I've seen where their results are concerning at first.
Over time this is what happens:
Blood Panel On Keto | What will it do? |
---|---|
Triglycerides | Consumed for fuel and therefore is reduced. |
VLDL-C | Decreases due to triglyceride shedding. Converted into IDL & consumed. |
LDL-C | Increase due to VLDL catabolism. |
LDL-C | Decrease. SD-LDL due to lower triglycerides. |
HDL-C | Increase. Contains primarily cholesterol so increased count as triglycerides drop. |
Total Cholesterol | Increase due to HDL-C & LDL-C rising. Could go either way. |
TC/HDL-C Ratio | Decrease. May go up if LDL-C rises significantly. Look to HDL-C for further analysis. |
Trig/HDL-C Ratio | Decrease. This should always improve. |
So:
Trigs, VLDL-C, LDL-C, TC all dropped.
Your ratios increased due to the drastic drop in HDL-C. Given the entirety of your profile I would keep testing every 3 months to graph out your progress.
As you can see a lot is improving here. Your body has a ton of cholesterol to move around and you are likely insulin resistant. Stay positive and strong and you'll see this all increase.
As far as HDL-C:
It did drop significantly but you've done a lot to your body right now and you will be in flux.

What this picture shows is that as your LDL particles are transformed into HDL particles they primarily enter small HDL particles. This causes a rapid flux of the cholesterol into VLDL particles. Over time as your LDL continues to decrease you may see the HDL continue to decrease initially but once the LDL is low enough the HDL particles will be able to fully mature and this number goes back up!
Hell your numbers are so different than norm that the equation I used was for your table was the one used for when trigs are below ~97 mg/dL!
