My new doctor has had it up to ✨here✨

Not in my experience. I had a GP recently go through 10 years of medical history and fixate on depression/anxiety/bing drinking from severe socioeconomic stress 6 years prior. Her notes are incredibly misleading and blatantly served to paint a picture of me as a person struggling with severe untreated mental health issues. Her notes state I don't need cholesterol or diabetes screening until I'm 40, high cholesterol and high fasting blood sugar are both documented in my history, I had prediabetes that is now full blown diabetes. She states that I "claimed to have PCOS but it is undocumented". It is well documented and I have lost an ovary to ovarian cysts. She also listed that I live with my husband and that I am unemployed, both are untrue. My favorite note she wrote was "wearing multiple layers of clothing and in acute distress". I have no idea what that even means, I was slightly tearful because I felt she was dismissive and I was wearing a sweater in MARCH. The absolute worst is she listed "illicit substance abuse" when I STUPIDLY admitted to smoking cannabis socially maybe 3-5 times a year in a legal state. She blamed all of my symptoms on recreational cannabis. These notes follow me everywhere, I was dismissed so many times in the ER because of her notes. It was very very obvious she never thought I would have access to them. I'm currently pursuing malpractice because I was later diagnosed with 3 different autoimmune conditions,amyloidosis and developed diabetes while under her care that she refused to test for; all of the symptoms were there she just didn't chart them focusing instead on incriminating me. It's very very difficult and the law is not on our side.
