Pretty sure I am trans. Now what?

Generally most go in the direction of getting hormones, but you don't have to if you don't want to. To do that you'd find a clinic/doctor that allows informed consent, you sign off on the paper that acknowledges that you know the risks and the effects and what and what not to expect, and then you get your hormones that day or that week.

Or you could go the route of gender therapist. You can search therapists online. It should say they specialize or at least work in areas of gender identity / transgender clients, and it should also say they give HRT referrals. You could call many of them straight up and say "I'm simply looking for an HRT referral to get my hormones, I'm willing to pay" or you could follow their sessions and what not. It's up to you.

Next, start taking care of yourself. Eyebrows, shaving. Get into laser or electro if you think you'd need it. Wait for the hormones to fuck you up good while growing your hair, perhaps planning on what clothes to buy, deciding on a hairstyle you want to aim for. Pick out a name that really calls to you.

It gets easier and more natural as you get into it. You'll realize that you don't really need a transition plan to make it.
