Well, nurse here. I’m not blaming her, but unintended pregancies are common with the pill form due to user error- again, not blaming her. It’s very common to not take the schedule dose at the same time every single day and totally understandable. With that said, the rate of success for the pill is solely based on perfect use. No perfect use, then that rate plummets. Who knows if that’s the reason though, but switching is something to consider.

If she’s still on birth control, I highly suggest she switch to a more effective method that requires less maintenance, such as the implant or IUD (I personally recommend the implant) to avoid a recurrence.
