Hey, I just thought I'd comment because I also have peyronie's disease (got it when I was 27 in the first relationship of my life only 2 months after losing my virginity, I'm 29 now). I had really bad pain while flaccid for 9 months that finally went away, but everytime I have sex or masturbate it comes back for a day or two. I'm still in the relationship that I was in when things started, and similar to you, I'm able to be sexual in other ways, although in my case I only have mild pain on erection unless there is friction on my penis or I ejaculate, in which case the pain because horrible. I can say that doctors are completely worthless when it comes to this condition, especially with pain.

I have been in contact with some others who have similar pain and there are a few things people have had some amount of success with. Since I don't know exactly what type of pain you have I can't say if any of these things will help or not, but I might as well tell you. First of all, some people take oral medications for nerve pain. The main ones that have been recommended are lyrica, clonazepam, and cymbalta. Personally I refuse to take any oral medications because of cognitive effects. I don't know how you feel about them. I have also had a compound cream recommended to me, which is 5% lidocaine, 5% amytriptaline, and 2% ketamine. You would apply to the penis twice a day or before anything sexual. I tried this and found it did help some with pain, although I got systemic side effects so I stopped. I did get this steroid injection. I think the study is sort of "too good to be true" but I do think it may have helped me. I did have increased pain for 3 weeks afterwards though, so just know that it isn't completely risk free. After this my pain decreased a lot, but this may have been natural recovery as things were finally starting to improve shortly before I got the shot.

The last thing I'll say is that I had a lot of pelvic pain after injuring my penis from chronically tensing the pelvic muscles, which is natural when you have pain in the penis. Maybe you don't have this issue, but if you do, seeing a pelvic pain physical therapist and learning to relax your pelvic muscles can help a lot. It's something I'm still dealing with.

Don't worry, I don't think 42 is old. That's young in my opinion. This condition is hard enough for guys who are in their 60's and older. It's hard at any age; nobody wants to stop having sex, I don't think it matters how old you are. But of course I still think it's tougher for us younger people.
