I totally understand where you're coming from. I think I'm going to be changing providers soon too because of this weird situation that happened to me. I have 3 more lined up, but seriously I know this sounds weird, I have to go to a secure location to call them so specific people know nothing about it. And I'm honestly not worried about not getting my meds, I have a comprehensive diagnosis and I have a TOVA test that said I had it 2 years ago as well. The last 2 doctors I talked to said they did TOVA but it would cost $150. I REALLY don't want to have to pay that, because I might be quitting my job soon. I just got a really good offer in Tunkhannock but the facility doesn't open for 2 months. However, it would give me time to just focus on the end of my semester. Anyway, I'd have to have the TOVA test records faxed if I want my meds to continue. But you're right, it's soooo irresponsible to do a self-response questionnaire. Waaay more irresponsible than not citing your sources. Like when you email someone you thought was a friend an article, and they pretend the words are theirs! LOL-why wouldn't they think you had an ethical duty to say something? Oh, right, because they have no ethics...
